1 cup unsalted shelled pistachios 0.5 cup granulated sugar 1 cup water 1 cup heavy cream 0.5 cup milk 1 tsp vanilla extract 0.25 tsp almond extract 0.25 cup pistachio vodka optional
In a saucepan, combine the pistachios, sugar, and water. Bring to a simmer over medium heat, stirring until the sugar dissolves, about 5 minutes.
Transfer the mixture to a blender or food processor and blend until smooth.
Strain the pistachio mixture through a fine-mesh strainer or cheesecloth into a mixing bowl to remove solids.
Stir in the heavy cream, milk, vanilla extract, and almond extract. Add pistachio vodka, if using, and mix well.
Chill the cocktail mixture in the refrigerator for at least 1 hour before serving.
